An electronic reporting (eReporting) option is now available through the NASA Safety Reporting System (NSRS). An electronic submission form is now available for those inside the NASA firewall (e.g., civil servants, contractors) with access to the NASA intranet. The paper form traditionally used to submit NSRS reports is still available for those who can’t report electronically or wish to maintain complete anonymity.

An updated and revised NASA Safety Reporting System (NSRS) course is now available in SATERN. The NSRS is the agency’s top-level anonymous reporting system for hazards and safety concerns and allows NASA employees and contractors to report concerns to upper management without fear of reprisal. 

NASA recently released the Human Factors Annual Report, which provides Human Factors analysis and details its impact on the agency. The report includes key mishap data for every NASA center, trends for the agency, a featured case study, goals and recommendations for the future, and Human Factors accomplishments.
